DAS Health will offer LinkedMD’s comprehensive CCM program to their clients including physician practices, medical groups, hospitals and Accountable Care Organizations.
LinkedMD provides CCM tailored to those services encouraged by Medicare through CPT Code 99490. As a LinkedMD value added reseller, DAS Health will educate physicians and their practices on the benefits of chronic care management as well as implement LinkedMD’s CCM program.
