MOORESTOWN, N.J., Dec. 14, 2016 -- Tabula Rasa HealthCare, Inc.’s (NASDAQ:TRHC) Medication Care Management Division and Mediture, a technology provider of Electronic Health Records (EHR) used by PACE organizations across the country, announced the official launch of their collaboratively developed, custom integration that enables a single electronic record, while accessing TRHC’s proprietary Medication Risk Mitigation (MRM) Matrix®, at the point of e-prescribing.
PACE (Program of All-Inclusive Care for the Elderly) is a Medicare and Medicaid program that helps eligible people, who are 55 or older, meet their health care needs in the community instead of entering a nursing home or other facility. Since 2011, TRHC has focused on optimizing outcomes for PACE through its national medication management and personalized, reminder adherence packaging and pharmacy services division. TRHC partners with approximately 15-20% of the PACE market nationwide.
