ALBANY, N.Y., June 24, 2016 -- House calls are making a comeback thanks to a new local business that is transforming care for thousands of local patients. Landmark Health is providing in-home, concierge-level care to Capital Regions residents with multiple chronic conditions.
Landmark clinicians are on-call 24-7 and guarantee a response time of less than one hour. The team consists of physicians, nurses, care managers, social workers, pharmacists, mental health experts, and dietitians.
"The Landmark team of mobile clinicians acts as the eyes and ears in a patient's home, collaborating with primary care doctors and specialists on individualized care plans. We are providing these services at no out-of-pocket expense to the patient with the ultimate goal of reducing costs by keeping patients out of the hospital." said Landmark CEO, Adam Boehler.
It is well-documented that spending for health care services in the U.S. is concentrated among a small percentage of people with high utilization.
