AUSTIN, Texas, July 18, 2017 -- Iris Plans and Brookdale Senior Living (Brookdale) today announce a new partnership to deliver Iris Plans’ telehealth Advance Care Planning (ACP) services to Brookdale residents.
The Advance Care Planning (ACP) services offered through Iris Plans - services also known as “healthcare planning” - empower Brookdale to continue and enhance its mission to nurture and improve the lives of its senior residents and their families. ACP is proven to reduce stress for both individuals with serious medical conditions, as well as their family and caregivers, while improving health outcomes.
“We are thrilled to welcome Brookdale Senior Living residents as members of the Iris Plans family,” said Steve Wardle, CEO of Iris Plans. “This new partnership allows us to help further Brookdale residents’ involvement in their own healthcare decisions, as well as provide them a platform to include family and loved ones in their decisions.”
Iris Plans’ tech-enabled healthcare planning services also assist Brookdale in their efforts to reduce hospital readmissions and the stress often felt by patients during these episodes.
“Advance Care Planning through Iris Plans is a great compliment to the many patient-centered services we offer at Brookdale Senior Living. Iris Plans will further enable Brookdale residents to connect and with their families and loved ones, and will help our residents plan for healthcare decisions, reduce stress around these decisions, and help avoid difficult and unnecessary hospital stays,” said Todd Mackenzie, Executive Director and Austin Market Leader for Brookdale Senior Living.
Through this new partnership, Iris Plans offers innovative and industry-first videoconference discussions between Iris Plans facilitators and Brookdale residents, allowing these residents to receive these valuable healthcare planning services from the comfort and safety of their Brookdale residences. Iris Plans empowers Brookdale residents to create a plan that guides important and complex decisions. Iris Plans securely distributes residents’ Advance Care Plans to anyone they choose, including families, doctors, and hospitals.
