Create offers a brand new health care option that increases health care quality and immediately reduces health care costs for plan sponsors, members, and their families. Through Create’s innovative enrollment platform, families are able to choose to receive care from a specific network of doctors and hospitals. This groundbreaking model enables families to develop strong, two-way relationships with health care providers within that group, resulting in better care. Create fully aligns the interests of plan sponsors, patients, families, physicians, and hospitals, delivering significant savings to plan sponsors and families. Create is available to plan sponsors now.

Mather Memorial Hospital, Mercy Medical Center, Saint Catherine of Siena Medical Center, Saint Charles Hospital & Rehabilitation Center, Saint Francis Hospital - The Heart Center, Saint Joseph Hospital, and South Nassau Communities Hospital. Families who choose to receive care from LIHN will also be able to work with affiliated physicians who contract separately.
