MOORESTOWN, N.J., Aug. 03, 2017 -- Tabula Rasa HealthCare (NASDAQ:TRHC) President and Chief Marketing and Business Development Officer, Orsula V. Knowlton, PharmD, MBA announced TRHC subsidary company CareKinesis has been selected to provide its comprehensive Medication Risk Mitigation by CareKinesis® and PACE-focused pharmacy services to PACE of Triad, Greensboro, NC. Implementation is scheduled to begin during the month of September for an October 1, 2017 start of services.
According to President Knowlton, this is the fourth PACE organization in North Carolina to select CareKinesis to serve as its pharmacy partner. It is the third largest program in the State with its nearly 200 participants. “We welcome PACE of the Triad to our client portfolio and are delighted to be its medication management partner, helping to assure optimal medication-related results,” said Knowlton. “We consistently strive to reduce medication-related risk while documenting improved economic, clinical and humanistic outcomes.”
PACE of the Triad, is a non-profit health and human services agency serving Guilford and Rockingham counties. A Program of All-Inclusive Care for the Elderly (PACE) coordinates and provides all needed preventive, primary, acute and long term services including medication management. This unique model of care provides all the care and services covered by Medicare and Medicaid, as authorized by the health care team. PACE also covers additional medically-necessary care and services not covered by Medicare and Medicaid if the health team determines the care and services are needed. PACE enables individuals who are 55 years old or older and certified by their state to need nursing home care to live as independently as possible for as long as possible.
PACE organizations nationwide rely on CareKinesis to increase medication adherence and reduce participant hospitalizations and recidivism. CareKinesis accomplishes this through prospective, geriatric-trained and board certified pharmacist medication management services in collaboration with PACE prescribers, a proprietary electronic medication platform, adherence packaging choices, and flexible medication access options.
